Terms and Conditions – Dr Adam Woo – March 2020

Dr Woo has consulting rooms at the venue shown below:

He aims to provide the report within four weeks of the appointment date and achieve this target in the great majority of cases. If there is special urgency ie 2 weeks or less this can usually be accommodated.

A standard Claimant Report with 250 pages or less takes about 6 to 8 hours typically. More complex cases or pages in records may incur slight increased charges if at all, please inquire.

Home visits can be arranged and are costed on receipt of instruction. The above costs are subject to increment depending on the volume of documentation and the complexity of the case.

Supplementary work is charged per hour.

Payment terms are 30-60 days from the date of the invoice. No additional work will be undertaken until the report fee is cleared. Any deviation can be requested.

All related medical records must be received by us prior to appointment date, or on commencement of desktop reporting, preferably electronically.

If a draft report is required prior to the receipt of full records, we reserve the right to charge additional fees due to the extra work of subsequent integration of additional information.

Court fees are charged daily.

Court Cancellation fees – 1 week- full, 2 weeks- 75%, 3 weeks- 50%, 4 weeks -25%

If a case settles whilst Dr Woo is in correspondence, any fees relating to work undertaken prior to notification will still be due.

Medical records will be kept for 6 months following first appointment date with Dr Woo. If you would like medical records returned, please advise on completion of report.

Please confirm your agreement with the above terms and conditions and I attach a copy of Dr Woo’s CV for information.

If acceptable, an appointment can then be given to the Claimant.

Should the claimant cancel an appointment without more than two weeks’ notice, there will be a cancellation fee
